- Registration Requirements:
  
  Sacrament preparation does not take place in parish Faith Formation classes or in the Catholic Schools. It is a separate and distinct program.
  Download registration form below and email completed form to [email protected] along with a copy of your child's baptism certificate (even if your child was baptized at Epiphany).
  Complete payment below.
  At least one Catholic parent must attend a mandatory Parent Sacrament Meeting to begin your student's Sacrament Preparation process. (Parent meeting dates are included in the document above this section.)
  Families must attend weekly Mass to give integrity to this sacrament preparation process.
  
  STEP 1: Student must currently be enrolled in Faith Formation classes or attending Catholic School.
